Notice of Privacy Practices

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

Insurance For All, LLC (“we,” “us,” or “our”) is committed to protecting the privacy of your health information. To the extent we receive, create, or maintain Protected Health Information (“PHI”) in connection with health, life, dental, vision, Medicare, or supplemental insurance quoting and enrollment, we follow the privacy and security requirements of the Health Insurance Portability and Accountability Act of 1996 and its implementing regulations (collectively, “HIPAA”).

1. Our Pledge Regarding Your Health Information

We understand that information about you and your health is personal. We are required by law to:

  • Maintain the privacy and security of your PHI;
  • Provide you with this notice of our legal duties and privacy practices regarding PHI;
  • Notify you in the event of a breach of unsecured PHI; and
  • Follow the terms of the notice currently in effect.

2. How We May Use and Disclose Your Health Information

We may use and disclose your PHI for the following purposes without your additional written authorization:

  • For Treatment, Payment, and Healthcare Operations: to facilitate enrollment in a health plan, process applications, and assist with claims, billing, and policy servicing.
  • To Insurance Carriers: we share PHI with carriers and underwriters you authorize so they can quote, issue, or administer health-related coverage.
  • Business Associates: we may disclose PHI to vendors that perform services on our behalf (such as quoting platforms or agency management systems) under written agreements requiring them to safeguard your PHI.
  • As Required by Law: including disclosures to health oversight agencies, in judicial proceedings, to law enforcement, or to avert a serious threat to health or safety.
  • Public Health Activities: such as reporting communicable diseases or product recalls, where required by law.

3. Uses and Disclosures Requiring Your Authorization

The following uses and disclosures will be made only with your written authorization:

  • Most uses and disclosures of psychotherapy notes;
  • Uses and disclosures for marketing purposes;
  • Disclosures that constitute a sale of PHI; and
  • Other uses and disclosures not described in this notice.

You may revoke an authorization in writing at any time, except to the extent we have already relied on it.

4. Your Rights Regarding Your Health Information

  • Right to inspect and copy the PHI we maintain about you, subject to limited exceptions.
  • Right to amend PHI you believe is incorrect or incomplete.
  • Right to an accounting of certain disclosures we have made of your PHI in the six years prior to your request.
  • Right to request restrictions on certain uses and disclosures of your PHI. We are not required to agree to all requests.
  • Right to confidential communications at an alternative address or by an alternative means (e.g., a personal email rather than a home address).
  • Right to a paper copy of this notice at any time, even if you have agreed to receive it electronically.
  • Right to be notified of a breach of your unsecured PHI.

5. Safeguards

We maintain administrative, technical, and physical safeguards to protect PHI in our possession, including access controls, employee training, encryption of electronic PHI in transit and at rest where appropriate, and ongoing risk assessments.

6. Changes to This Notice

We reserve the right to change this notice and to make the revised notice effective for PHI we already have about you, as well as any information we receive in the future. The current version will always be posted on our Site with its effective date.

7. Complaints

If you believe your privacy rights have been violated, you may file a complaint with us using the contact information below or with the U.S. Department of Health and Human Services, Office for Civil Rights at www.hhs.gov/ocr/privacy/hipaa/complaints. We will not retaliate against you for filing a complaint.
